2021 Senior Tribute Questionnaire

Hey Seniors!

The Sentinel invites members of the St. Joseph-Ogden and Unity High School Class of 2021 to fill out the form below to enjoy their own personal profile for our 2021 Salute to Seniors.

Here is a sample of what we did last year for seniors in our area. (Please excuse the page formatting. The Sentinel site has undergone a redesign since these pages were originally created and may not appear as elegantly as they did originally.)

There is no charge to be included. 

The Sentinel wants to help share and celebrate your accomplishments and highlights from your high school career with readers in our area.

You can copy and paste the form below into an email app and send it to editor@oursentinel.com or print it out and send by mail to: PhotoNews Media, 136 West Main, Urbana, IL, 61801. You may attached or enclose a photograph. Please tell us the name of the photographer who took the photo so that we can credit them properly and ask for permission to publish their photo.

Your profile can be submitted starting today and we'll take them until 6pm on Saturday, May 31. We will publish as many as we can a day starting on April 18, 2021.
